With deep expertise in GenAI capabilities, Ascendion created a web application that analyzes high-risk patients and, based on that analysis, automatically generates a care plan complete with goals, outcomes, interventions, and both long-term and short-term milestones.
To power that intelligence, Ascendion built the company’s own healthcare knowledge base and embedded it into vector databases like Pinecone. The solution brings together Azure OpenAI LLM models, prompt engineering techniques, and vector database integration to generate care plans that are both clinically grounded and genuinely personalized to each patient.
